Medical Assessment

Our experienced doctors will perform quick & simple step by step assessment which will include

• Noting any relevant current and previous health issues. Please noted we do not have access to your medical records and hence bring relevant document or answer the question to best of your ability
• They will perform physical examination, eyesight test and blood pressure check.

Important Information for drivers attending with the D4 Form Only

1. You must fill out our Consent Form (please link it here so that they can click and open the link) and take it to your registered GP prior to your appointment.
2. A copy of your medical records form your registered GP must be brought during your appointment. Attending without a copy of your medical records may result in a delay in your form being released.

What to bring for your medical examination assessment

1. You must bring either your D4 form or relevant Taxi Medical form for the appointment. (Failure to bring your form may result in booking another appointment and additional charges).
2. You must print your name (do not sign your name) and write your date of birth in the indicated boxes on the bottom of every page of the D4 form.
3. You must complete your personal details and your NHS GP details on the form.
4. If you require glasses for driving, you must bring them with you for the eyesight test along with your eye test prescription. This must be dated within the last 12 months. If you are wearing contact lenses then they must be removed during the eyesight test.
5. If you are or have recently undergone any hospital treatment or you are under the care of a specialist, then you must bring details with you including Dates of attendance, hospital report or letter and results.
6. You must bring a photo I.D. (passport or photo driving license only). Without photo I.D. we will not be able to release your medical forms
